Queen Square Family Health Team Partners with Community Physicians to Support Ontario’s Primary Care Action Plan
Brampton, ON — Queen Square Family Health Team (QSFHT) is launching a collaborative partnership with community physicians to help attach more Brampton residents to family doctors and comprehensive primary care services. This work directly aligns with Ontario’s Primary Care Action Plan, which aims to ensure that every person in Ontario is connected to a family doctor or primary care nurse practitioner.
Ontario’s plan emphasizes expanding publicly funded primary care teams, improving access to comprehensive services, and reducing reliance on high-cost areas of the healthcare system. One of its core goals is increasing patient attachment by creating and expanding primary care teams across the province, supported by a historic $1.8 billion investment intended to connect two million more Ontarians to primary care within four years.
Queen Square FHT’s new collaboration reflects this provincial direction. As part of the initiative, community physicians will expand their patient panels, enabling more individuals who currently lack a primary care provider to enroll with a primary care team. Once enrolled, these patients will gain access not only to their family doctor but also to Queen Square’s full range of interdisciplinary supports.
This includes services such as:
- Occupational therapy services
- Mental health services
- Community pharmacy services
- Palliative care
- Dietitian services
- Maternal and child health program
- Chronic disease management programs
- And many more
These services support the province’s vision of primary care that is comprehensive, convenient, and connected. Strengthening access to community-based care also aligns with evidence outlined in Ontario’s initiative, which highlights that primary care reduces emergency department use, improves health outcomes, and enhances system efficiency.
By partnering with community physicians, Queen Square FHT is contributing to the Action Plan’s first pillar, Connecting You to a Primary Care Team, which focuses on expanding interdisciplinary teams and attaching hundreds of thousands of people each year. The initiative also supports provincial objectives related to equitable access, timely care, and improved patient navigation.
