Service Description: Provides:
  • education to enhance knowledge and skills of staff
  • consultation about learning need
  • care plans and programs related to challenging behaviours of clients / residents
  • assistance with referrals to services and resources and enhanced development of linkages between services providing dementia care
Consultant to:
  • clinical and administrative staff in long-term care facilities
  • community service agencies and Home and Community Care Support Services Central West
  • Adult Day Programs
  • Homemaking agencies
  • Supportive Housing
  • Respite Care Programs dealing with dementia and mental health issues

Fees: None - Covered by OHIP
Application: Referrals can be made by anyone; within a government funded community agency; who has identified a learning need related to the mental health of the elderly; or anyone concerned about an older adult who may have a psychological, behavioural or memory problem
Eligibility / Target Population: Must be a government funded agency such as: Home and Community Support Services; Adult Day Programs; Homemaking agencies; Supportive Housing; Respite Care programs dealing with dementia and mental health issues
