Service Description:

Network of partners providing service coordination in Peel through the following community agencies including Brampton Caledon Community Living, Child Development Resource Connection Peel, Community Living Mississauga, ErinoakKids and Kerry’s Place Autism Services.
The partners in Kids Pathways Peel have joined together to embrace a simplified approach to service coordination that assists children, youth and their families to strengthen their capacity to care for their child/themselves and enhance their formal and informal networks of community resources.
Kids Pathways Peel provides resource coordination for children and youth from 0-18 with special needs, living in Brampton, Caledon, and Mississauga. Kids Pathways Peel will support families in planning services for their children. Once families/individuals are registered with Kids Pathways Peel, they will receive specific information about programs, respite services, workshops and other resources that may be of interest. This information will be distributed on an ongoing basis and will vary depending on the age and needs of the individuals.
This service will work closely with individuals and families to anticipate the need for support during key transitions. Information, Referral and Resource Specialists will respond to a family’s questions and/or concerns, and provide timely support and guidance. Urgent concerns will be screened, triaged and referred where necessary to community partners within Kids Pathways Peel for more intense service coordination.
