Service Description:

Community based care and hospice residence care to people and their families touched by life-limiting illness
Hospice residence care is provided by a team of palliative care professionals when life expectancy is three months or less
|

Community services: Referrals will be accepted from a person requiring service, physicians, family members, friends, spiritual and religious leaders and health care professionals * people being referred to the program must have approval from the Power of Attorney and accept the services based on the established criteria * admission is based upon medical, physical, emotional and spiritual need Residential services: Contact Ontario Health atHome-Brampton * information and tour of Bethell House can be made by calling Bethell House directly
