Service Description: Adult day program * social, recreational, and therapeutic activities to physically frail and cognitively impaired seniors and adults in a group setting * allows caregivers who provide day-to-day care to receive some respite during the day to rest, recharge and complete everyday tasks * individual interests and hobbies are incorporated into the program * snacks and meals are tailored to meet dietary restrictions * clients must be independent with mobility * no walkers or wheelchairs in program room * transportation is not included in program fee, but is available upon request for an additional fee (based on availability)

Accredited with Exemplary Standing by Accreditation Canada

Fees: $35 per day * subsidy available for eligible participants * additional fee for transportation

Eligibility / Target Population: People with Alzheimer's Disease or related dementias * Ontario Health atHome Care Coordinators assess the needs of applicants
