Service Description: Speech-language pathologists, physiotherapists, occupational therapists, and registered dietitians with extensive clinical experience in adult and paediatric rehabilitation
 
Dietitian Services - Registered Dietitians (RD) providing dietetic services in the community * provide assessments and treatment for people of all ages * can help with, meal management, diabetes, weight loss, palliative support care, allergies, tube feeding, therapeutic intervention (renal failure, Cardiac disease, Parkinson’s disease, Multiple Sclerosis and Cerebral Palsy), dysphagia and transitional feeding
 
Occupational Therapy - Home-based occupational therapy via specialty teams including paediatrics, neurological disorders, acquired brain injury and school health support * treat mobility issues, fine motor and sensory processing issues and wheelchair and seating issues
 
Physiotherapy - Home-based physiotherapy services via specialty teams including paediatrics, neurological disorders, acquired brain injury and school health support * assist with improving functional independence, gross motor development, prevention and management of pain, prevention of physical impairments and disabilities and in general promote fitness, health and wellness
 
Speech Language Pathology - Speech language pathology services to clients of all ages * intervention is offered for speech disorders * oral motor skills * language * communication interaction * cognitive communication, feeding * swallowing disorders

Application: Clients are usually referred by Ontario Health atHome
Eligibility / Target Population: Clients are usually assessed by a Ontario Health atHome care coordinator for eligibility * Clients and health care professionals can contact 1to1 directly to inquire
