Applications managed by Services and Housing In the Province (SHIP)
Referrals accepted from family physicians, hospitals, partner agencies, family members and significant others or applicants
Requirements: Signed Consent to Disclose Personal Information Form (Original, not copy or fax) has to be submitted before application can be processed
Complete SHIP application form
A psychiatric report to be submitted to Central Intake within six months of the date of application submission
