Service Description:
|
Basic to advanced level diabetes education and ongoing care to children and families, youth and adults who are at risk or have Type 1 diabetes, pre-diabetes, Type 2 diabetes, pre-gestational diabetes, and insulin pump starts and management. Our program consists of a multidisciplinary team including Registered Nurses CDE, Registered Dietitians CDE, Social Workers and physicians
Services are available for:
- Pre-Diabetes
- Type 2 Diabetes
- Type 1 Diabetes
- Paediatrics (children)
- Diabetes in pregnancy and gestational diabetes
- Insulin Pump Therapy
Location: On the lower level of HHCC – down the same hallway as the cafeteria

Appointment available on a self-referral with the exception of specialty clinics * endocrinology and pediatrics require a physician referral
