Service Description:
|
Supports outpatient diabetes management for both type I and type II diabetes (children and teens up to 18 years of age) * clinical team including pediatricians specialized in providing diabetes care, registered nurses, dietitians, social workers, child life specialist, and certified diabetes educators to help support families and their children/teens learn to live well with diabetes * can test HbA1c (90-day average blood sugar) onsite
Services include:
- assessment and treatment of pediatric diabetes (type I and II)
- medical interventions
- resource planning and support
- nutrition counselling
- diabetes management and education
- insulin pump program
- focus on mental wellness and coping
- Assistive Devices Program (ADP ) on-site centre to help patients applying for government funding to cover most of the cost of insulin supplies
