Service Description: Program for seniors experiencing serious and persistent mental health concerns which include: depression, mood changes, dementia, responsive behaviours, and/or anxiety

Seniors having challenges accessing supports due to geography, isolation or changes in informal support system are considered a priority for this service
  • makes referrals to, and collaborates with, community partners such as Ontario Health atHome-Brampton (Central West), Seniors Day Programs, Alzheimer’s Society of Dufferin and local hospitals
  • education and support to the client and care team, to ensure clients have the necessary services and supports to live successfully in the community and to maintain/improve quality of life

Application: To complete a referral call 905-451-2123 * referrals received from SGS or Headwaters Hospital GEM nurse
Eligibility / Target Population: Seniors with mental health issues
Languages: English
Language Notes: Translation and interpretation is available for most languages. When you call please let us know if interpretation services are needed
