Service Description:

Inpatient consult service is available to patients admitted to hospital and their families
Offer:- recommendations to optimize pain and other symptoms
- information around what to expect, discussions regarding options, and establishing goals of care
- transfer to the acute palliative care unit for management of complex symptoms or end of life care (when indicated)
- support with planning for palliative care at home or other facilities
- follow up in outpatient clinic
Location: 5th floor, North Building, Entrance A, take the ROSE elevators
