Service Description:
|
Counselling and support for people with type 1 diabetes and type 2 diabetes (on insulin or insulin pump), gestational diabetes * help to patients and families to understand diabetes and how to self manage the disease - test blood sugars, take insulin, manage medications, make healthy food choices
- appointments conducted in person virtually or by telephone
- insulin pump program
- insulin adjustment classes (pattern management/30/70 class)
- gestational diabetes program transitional program for young adults moving from a pediatric program
- lipids class
- healthy kidney class
- weight management program
- satellite diabetes education centres (Bolton, Malton and Woodbridge)
- South Asian programs (Peel and Etobicoke)
- referrals to social worker and chiropodist (foot clinic)
- community diabetes seminars
- in-services to health care providers (hospital and community)
