Service Description:

Resources to help those in need of health and wellness, who are in medical or mental distress
Partnerships with primary care providers, including Homeless Health Peel and Wellfort’s Bloom Clinic team, and Diabetic Nurse, provide clients with ongoing medical support. These teams are hosted on a weekly basis during regular hours of operation
