Service Description:

Assistance to people experiencing complex and overlapping issues with rapid, low barrier support to address substance use, mental health and homelessness challenges * service delivery through a collaborative network of clinical, social service and care providers working together to connect people with the supports they need
Services offered:
- onsite and outreach support for mental health and addiction
- housing help
- service information and navigation
- counselling
- basic needs support (food, hygiene products)
- nursing care
- transitional and supportive housing
- employment support
- peer support
- foot and wound care
- dietary support
- mobile clinic services
Main office is in Orangeville, and a satellite office in Shelburne
