Service Description:
|
To avoid caregiver burnout, hospital emergency department visits, unnecessary hospital admissions and early admissions to Long-Term Care facilities, among older adults, the overnight respite program offers in-home respite to clients that qualify for the program. Service provided by Overnight Respite Workers (ORWs)
The program is not equipped to support bedridden or palliative clients

Eligibility / Target Population:
|
Older adults aged 55+ Reside in Brampton or Malton (Mississauga) Recent discharge from hospital (within 1 week to 2 months) Diagnosed with Alzheimer's or mental illness requiring emotional support older adults who have caregivers experiencing burnout
