Service Description:
|
A multidisciplinary team that specializes in caring for Seniors in the community. The team provides in-home non-urgent assessments to frail seniors that are unable to attend outpatient clinic appointments. The assessment focuses on functional, psychological, cognitive and physical health
Patient exclusions:
- clients residing outside the Ontario Health atHome, Brampton (Central West) geography
- clients with no family physician
- clients under 65 years of age
Services offered:
The team will conduct an assessment as related to the needs of the patient and/or issues that are identified such as; frequent falls; mobility concerns, medication polypharmacy, pain management, weight loss/nutrition, cognition/dementia, delirium, depression, anxiety, wandering, lives alone, caregiver stress, elder abuse, social isolation, ADL/iADL decline, home safety

Physicians, inpatient units and ER/UCC can refer a patient to the GOT team * Physicians referring a patient to this clinic are requested to complete the associated referral form and fax it to the clinic for an appointment. For your reference, the fax number is included on the form

Community-dwelling seniors identified to be at risk of further decline in daily functional performance
