Service Description:

Osler Emergency Department includes GEM nurses. These nurses:- assess patients to identify their health, functional, emotional cognitive and social needs
- link seniors to community services
- provide education to patients, their families and Emergency Department staff
- support seamless transitions between the Emergency Department and community health care services
