The GOT team - multidisciplinary team (serving patients living in Ontario Health atHome, Central West area) that specializes in caring for seniors living in the community * in-home non-urgent geriatric assessments for frail seniors who are unable to attend clinic visits * assessments focus on functional, psychosocial, cognitive and physical health status
