Service Description:
|
Family practice medical services with healthcare providers multidisciplinary health care teams that include family physicians, nurse practitioners and registered nurses
Services include- one-on-one appointment for individual client
- Virtual services
- group services with specific themes and objectives for clients and community members (pre-registration is usually required)
- diabetes education program
- special health education activities and programs for clients and community members (pre-registration may be required)
- pregnancy care for women up to 32 weeks
- immunization for babies and young children
