Service Description:
|
Through the Community Health Worker, Rexdale CHC provides clients with:- information and advice about many of the social determinants of health, such as housing, immigration, social assistance, education and links to employment agencies
- assist clients with hospital support, referrals and supportive counselling
- provide assistance to prepare documents and can obtain translation and interpretation services in multiple languages as required
The Rexdale CHC offers its clients the supports they need through one-on-one assistance or through group programs. In partnership with local agencies and community groups, the Community Health Worker at Rexdale CHC can provide health education, support groups and interpretation
