Service Description: A community response to working with children/youth with complex and chronic needs within the mental health system. The objective of the program is to respond to children’s mental health service system gaps. The service is intensive and long-term

Intensive Resource Coordinators focus heavily on working collaboratively with other agencies and service partners in the local community and provincially. Coordinators look within informal supports and broader aspects of the community to support the individual as well. Interventions are individualized, efficient and flexible based on the complexity of the child/youth and the clinical goals identified by the active clinical team attached

Application: Call intake or fax in referral * Youth and caregivers can self-refer, or physicians and community professionals can submit a referral using a form available on the WhereToStart.ca website
Eligibility / Target Population: Children under 18 years with many serious difficulties with feelings, relationships and behavior * these difficulties have been happening for a long time * child diagnosed with two or more disorders * child having difficulty getting along at home, school and in the community * child is getting service or waiting for service from other community agencies * and the family is willing to work with the agency and other community services to help the child
