Service Description: Program for people with mental health and/or substance use issues that has limited or negatively impacted their ability to function on a daily basis for a prolonged period of time * supports independence and improves quality of life
  • establishes a supporting, trusting relationship with the person through regular visits
  • help person navigate the system and advocate for the services/supports they need
  • complete assessment of needs using OCAN tool to develop personal recovery goals
  • provide education about mental health, medications, coping strategies, etc. 
  • crisis intervention and planning to increase persons ability to manage future stressors

Eligibility / Target Population: Adults 18 years and over with mental health or addiction crisis or people having difficulty coping
Languages: English
Language Notes: Translation and interpretation is available for most languages. When you call please let us know if interpretation services are needed
