Service Description: This program provides specialized support and education to older adults presenting with or at risk for responsive behaviours that may be associated with dementia, complex mental health, substance use and/or other neurological conditions. Family caregivers and other care partners are also included in the population that BSO serves

Central Intake and Triage in the Community
The BSO team provides clinical triage assessments, safety and risk mitigation, and assistance with health system navigation for clients living in community settings

In the Community
Community services collaborate with health partners and specialized services to provide assessment and education, and to link people living with responsive behaviours and their care partners to appropriate and timely resources

In Long-Term Care Homes (LTC)
BSO staff are located within all long-term care homes in the Central West area. They collaborate with LTC staff to plan and implement person-centered approaches to meet the needs of resident’s experiencing responsive behaviours

A multidisciplinary mobile team enhances BSO support across all Central West LTC homes, strengthening services for residents with responsive behaviours and building staff knowledge and capacity in behavioural care approaches

In Hospital
BSO acute interventionists support patients and families during their hospital stay and ensure smooth transitions as patients move from the hospital to wherever they call home
