Service Description: Program launched by Dufferin County Paramedic Service, in partnership with Ontario Health atHome-Brampton (Central West) and Headwaters Health Care Centre, helps seniors, the disabled and people with chronic illnesses get the care they need * Non-emergency, community-based service with a focus on health promotion, system navigation and injury prevention

Serves high risk patients living with complex medical issues such as diabetes, COPD, CHF and co-morbidities

May provide the following services:

  • wellness checks
  • safety home assessments
  • ability to refer to community partners
  • in home blood work
  • urine tests
  • in consultation with primary care physicians, administration of medications for various conditions
  • remote Patient Monitoring (RPM)
