Service Description: Addictions supportive housing for individuals who are living with problematic substance use issues and who are high users of emergency services. Through the rent supplement program, singles and women-led households can access housing units and follow-up support. Units are available in dedicated or mixed-income buildings. Eligibility criteria: - severe and active substance use challenge - homeless or marginally housed - qualify for a housing subsidy under the criteria set by the Ministry of Health - high-intensity service use (emergency department visits, withdrawal management system admissions, hospitalizations, contacts with the justice system).


Application: Referrals are accepted from anyone, including self-referrals. Referrals are through The Access Point. Please call or fill out the application form online. Once the application is received, a service navigator will call the individual to set up an appointment to do an intake assessment.
