Service Description:

Help for patients to understand:

how COPD affects their lungs
medications and why, when and how to take them
what causes a COPD “flare up” and how to avoid one
when to seek medical attention

If patient is admitted to hospital, referral may also be made to an Urgent COPD Clinic upon discharge. This helps the patient get follow-up therapy, if needed
