Service Description:
|
In-home medical support to clients who:
- need help at home after being discharged from the hospital
- are on the wait list for a long-term care home or are eligible for long-term care
- have chronic health conditions
Treatments and procedures - administer:
- blood and urine tests
- steroids and bronchodilators for chronic obstructive pulmonary disease (COPD)
- antibiotics for urinary tract infections and chronic obstructive pulmonary disease (COPD)
- diuretics for heart failure
- laxatives and enemas
- intravenous (IV) and fluid therapy
- conduct bladder scans
Can also refer clients for additional tests and specialist care
