Service Description: The Clinic supports seniors at high risk of injury due to a fall. Assessments include interviews, questionnaires, standardized fall risk and frailty testing, physical examinations and cognitive screening, when appropriate

Conditions treated:

  • community based patients aged 65 year or over with a history of one or more falls of uncertain origin in the past year
  • those with a history of frequent near-falls or instability requiring assessment or identification of fall risk(s)
  • additional clinical signs of frailty; slower gait speed, weight loss, fatigue, weakness, sedentary life with/without cognitive impairment


Patient exclusions: patients who are wheelchair bound or use wheelchairs for their primary mode of mobility; patients for whom fall issues are primarily related to their home environment (home safety assessment more appropriate); patients who are unable to attend an outpatient clinic; patients who require assistance for all gait; transfers (refer to Reconditioning clinic); patients post-CVA who are eligible for the stroke program

Patients are assessed and receive a coordinated care plan from a team of experts
