Service Description: The Clinic supports seniors at high risk of injury due to a fall. Assessments include interviews, questionnaires, standardized fall risk and frailty testing, physical examinations and cognitive screening, when appropriate.

This clinic is appropriate for people aged 65 years or older who
  • have a history of falls
  • have been at risk of falls in the past year; or
  • show clinical signs of frailty (e.g. slower gait speed, weight loss, fatigue, weakness, sedentary lifestyle with or without cognitive impairment, etc.

Patients are assessed and receive a coordinated care plan from a team of experts

Fees: Most fees covered by OHIP
Application: Medical referral required
Eligibility / Target Population: Adults 65 years or older who are at high risk of injury due to a fall
