Service Description:

Standardized comprehensive geriatric assessments and care plans are offered to support seniors with complex care needs. Depending on needs, patients may benefit from one of the clinics
Care is offered by a team of health care professionals:
- geriatricians physicians specialized in the care of seniors
- nurse practitioners registered nurses
- physiotherapists
- occupational therapists
- speech-language pathologists
- social workers
- registered dietitians
- pharmacists
- team coordinators and clerical staff
