Service Description:
|
Social Work supports and services include:
- One-to-One Supportive Counselling
Topics may include:
- living with acute and chronic health challenges; coping with illness-related anxiety and depression; adjusting during times of transition; dealing with anxiety related to testing and procedures, and with fear of dying; facilitating legacy work projects
- managing caregiver responsibilities and avoiding burnout; dealing with anticipatory grief
- coping with bereavement, loss and grief
Additional Social Work services and supports include:
- Support Groups
- Assistance navigating the health care system
- Advanced Care Planning
- Patient Advocacy
- Guidance on funding sources and assistance with the application process
- Linkages and referrals to complementary supports
- Education and Resources
