Service Description:

Health centre focused on family and community health * health
professionals assess and treat non-life-threatening injuries or
illnesses * referrals provided to other local health services and personal development groups
Family practice medical services involve a multidisciplinary healthcare team that includes family physicians, nurse practitioners, and registered nurses. Services include:
- one-on-one medical appointment for individual clients, in-person and virtual visits
- interprofessional primary care for the African, Caribbean Black population
- well baby visit
- immunization for infants and young children
- adult health maintenance and management of chronic diseases
- pregnancy care for women who are up to 27 weeks pregnant
- mental health and substance use disorder services
- chronic disease management - diabetes education, physiotherapy, teleophthalmology-diabetes eye screening, foot care
- sickle cell disease
- sexual health clinic
- mental health counselling
- registered dietitian services
- case management and system navigation
- mobile health care (Wheels to Wellness) at specific locations in the community
- special health education activities and programs for clients and community members (pre-registration may be required)
