Service Description:

Clients and their families, through a guided journey, receive help to recover from substance use behaviors and related concerns. The program:
- supports clients who identify with having a substance use
problem or addiction
- reduces hospitalization-visits/admissions
- provides culturally competent psychoeducation to addiction clients and their families
- links to clinical care and
community resources
- creates care plan to suit each client’s
individual needs and preferences
- offers home visits and group sessions as required

- Long-term Case Management Program
- Sahara Bridging Addiction Counselling Program (BAC)
- Sahara In-Short Term Emergency Diversion
